Integrated chronic disease management (ICDM) online clearinghouse

Extensive consultation has highlighted the need to provide a sustainable and systematic way for agencies implementing chronic disease programs and initiatives to be able to access the largely unpublished resources, tools and reports of work produced by other agencies, particularly those specifically funded for chronic disease initiatives.

The Clearinghouse will be a resource to support the work of all Victorian clinicians and services pursuing integrated chronic disease management, including:

  • Early Intervention in Chronic Disease initiatives
  • Community health services
  • HARP-CDM programs
  • Primary Care Partnership staff and member agencies
  • Divisions of General Practice.

The ICDM online clearinghouse will allow practical resources and tools, plus brief explanations of their development and use, to be uploaded to a website by their authors where others can then search and access them. It is intended to reduce the duplication of and increase and expedite access to practical unpublished tools and resources for all health services, as well as reduce the growing burden upon those health services running well-established and innovative Victorian chronic disease initiatives by providing an efficient mechanism for the sharing learnings and resources.

ADMA plans to launch the ICDM online clearinghouse at their annual conference in Melbourne on 3 and 4 September 2009.
