Care Planning with General Practice - New Resource

This resource answers some frequently asked questions about integrated chronic disease management, specifically care planning for people with chronic disease and complex care needs that involves working with general practice and the Medicare Benefits Schedule (MBS) Chronic Disease Management (CDM) items. It covers the role nurses and allied health professionals, employed by a public health service, have in relation to care planning with general practice.

General Practice - PCP partnership delivers chronic disease improvements in Banyule and Nillumbik - New Case Study

Consumers, healthcare providers and health services in Melbourne's north-eastern suburbs are enjoying increased service coordination and integrated chronic disease management resulting from a strong partnership between the North East Valley Division of General Practice and the Banyule Nillumbik Primary Care Alliance (BNPCA), a Primary Care Partnership (PCP). A new case study describes the work that has been undertaken and the outcomes it is delivering for the community.

Industry Adviser Project extended to 2010

The Primary Health Branch is pleased to inform that the Integrated Chronic Disease Management Industry Adviser project has been extended until 30 June 2010. An evaluation of the first phase of the project has now been completed. A workplan for the next phase of the Industry Adviser project will be developed to reflect the recommendations from the project report, and feedback from recent consultations. Further input is welcome and should be directed to Ruth Azzopardi, ICDM Manager, Primary Health Branch.
