Extension of the Integrated Chronic Disease Management Industry Advisor Project 2009-10

Building on the learnings from the Integrated Chronic Disease Management (ICDM) Industry Advisor project's first phase in 2008-09 and the evaluation findings, the Industry Advisor model has been further developed and refined to have three distinct functions in 2009-10. The Industry Advisors will provide:
- targeted consultancy support to primary care partnerships and ICDM projects
- provide phone and email support for troubleshooting and sharing learnings
- undertake several state-wide capacity building projects.
