This unit has a focus on service system integration and development, both across primary health and between primary health and other human service sectors. It is responsible for:
- Primary Care Partnerships (PCPs)
- PCP and community and women’s health promotion
- Chronic disease with an emphasis on early intervention for those diagnosed with chronic disease
- 'Sustaining Community Well Being' – primary health drought response
- NURSE-ON-CALL and related health call centre developments
- The interface between state funded primary health services with General Practice to enhance and increase access for Victorians to primary health care.
