Indigo Assessment and Care Plan Coordination Service is managed by a consortium led by Western Region Health Service and includes Salvation Army St Kilda Crisis Service, HomeGround Services and North Western Mental Health.
Who is Indigo Assessment and Care Plan Coordination Service and how does it work
Indigo Assessment and Care Plan Coordination Service (Indigo) is a state wide service funded by MACNI to provide a range of services to clients of the MACNI as well as their service providers.
Indigo provides the following services
- Assessment
- Care plan development
- Care plan coordination
- secondary consultation
- training
- mentoring and co-work
Indigo works closely with regions to ensure that the plans developed for the MACNI clients are meeting their needs and sustainable.
