Assessment and Care planning

Indigo Care Plan Development and Coordination Service (Indigo) is managed by a consortium led by Western Region Health Service and includes Salvation Army St Kilda Crisis Service, HomeGround Services and North Western Mental Health.

Who is Indigo Care Plan Development and Coordination service and how does it work?

Indigo is a state wide service funded by MACNI to provide a range of services to clients of the MACNI as well as their service providers.

Indigo provides the following services

  • Assessment
  • Care plan development
  • Care plan coordination
  • Secondary consultation
  • Training
  • Mentoring and co-work

Indigo works closely with clients, regions and service providers to ensure that the plans developed for the MACNI clients are meeting their needs and are sustainable.
