An audience of 150 has heard how
partnerships are making a difference to Victorians with complex needs.
The Partnerships Making a
Difference forum highlighted the Department of Human Services Primary Care
Partnerships initiative and how cross-sector programs can work well.
DHS Secretary Fran Thorn said
Primary Care Partnerships facilitated integration across human services.
‘For clients, this has resulted
in early identification of their needs, not having to retell their story and a
coordinated service system response.
‘For service providers, this has
enabled efficiency and effectiveness gains in their service response and
improved collaborative planning, implementation and evaluation of initiatives.
‘For DHS programs and other
Government departments, this has meant harnessing a partnership of
organisations that has the capacity to deliver service system reforms in a way
that provides a mature infrastructure and statewide enablers.’
Rural and Regional Health and
Aged Care Services Division Executive Director Dr Chris Brook provided the
background to the Primary Care Partnerships strategy.
Department of Justice Office of
Gaming and Racing Acting Executive Director Micheil Brodie highlighted a
partnership with the Department of Human Services to respond to the need of
Victorians with problem gambling issues.
Whitehorse Community Health
Service Chief Executive Officer Jim Killeen presented on the work of Eastern
Metro Region Primary Care Partnerships implementing agreed systems of care
across organisations for people with type two diabetes.
Western Region Health Centre
Chief Executive Officer Clare Amies talked about building on existing service
integration through Primary Care Partnerships to support people with a refugee
background.
