Service Coordination Practice Standards

The "Victorian Service Coordination Practice Manual" defines the common practices, processes, protocols and systems which support Service Coordination across Victoria.

The manual includes a pull-out poster that describes the consumer’s path through Service Coordination and is supported by two resources: the "Good Practice Guide" and the "Continuous Improvement Framework".

Primary Care Partnerships may also have local protocols to complement the Victorian Service Coordination Practice Manual and information about these may be found on each of the specific PCP webpages.

Privacy and consent

  • Resources which aim to assist practitioners in their discussions with consumers regarding how their information will be dealt with.

Care Planning

  • Information to support the implementation of inter-agency care planning. Descriptions of projects, models and resources.
