Improving lives DVD

A community education DVD Improving Lives has been developed by Disability Services to illustrate the alternative support options available through my future my choice.

The focus of the DVD is to celebrate the positive impact that self directed planning and the provision of individual disability supports can have on the quality of life of younger people living in, or at risk of entry to, residential aged care.

The DVD has four chapters, outlining the key outcome areas of the my future my choice initiative:

  1. Planning and assessment
  2. Enhancing the support of younger people who remain in residential aged care
  3. Minimising new admissions of younger people to residential aged care
  4. Supporting younger people to move out of residential aged care
