Aids and Equipment Program guidelines and forms

Contact a prescribing therapist, usually an allied health or health professional, and arrange for an assessment to find out the most appropriate and cost effective aid or piece of equipment for you.

Fill out a Aids and Equipment General Application Form (see below) and submit it to your local Aids and Equipment issuing centre, along with a copy of the prescribing therapist’s assessment report.

Guidelines and forms

The Aids and Equipment Guidelines provide a framework for Victorian Aids and Equipment issuing centres and Department of Human Services staff for the operation of a range of aids and equipment programs.
