The allocation of ongoing disability supports, such as Individual Support Packages, accommodation and day programs, is managed through the Disability Support Register (DSR). The DSR is a system that records information about a person's current need for support so that when services become available they can be allocated in a fair, transparent and efficient way.
How the Disability Support Register works
The following guidelines provide service providers, people with a disability, their families and carers with information about how the DSR works.
The DSR Registration Guidelines provides information about how a person's application for ongoing disability supports is prepared and registered on the DSR.

Disability Support Application form:
The Individual Profile for Group Support form is to provide an overview of a person and their situation to assist in the allocation of group supports:
The Disability Support Application Review form should only be completed if a person already has an application registered on the DSR:

June 2009 Disability Support Register
The Department of Human Services’ approach to supporting people with a disability focuses on individually tailored and flexible ways to respond to people’s needs and assist them to achieve their goals and aspirations. This approach is in line with the goals of the State Disability Plan.
The number of people recorded on the DSR as at 30 June 2009 is shown in the following table.
