  • The Disability Services Commissioner will advise disability service providers of their complaints reporting requirements in the future – this template may be used to record complaints data until these requirements are published.

    It is envisaged that only deidentified data would be provided to the Disability Services Commissioner as part of the annual reporting requirement.
