About the disability services you get - Section 89 template form (Easy read) (PDF 1.0 mb)
- An interactive form template that can be filled out online about the services you get from your disability service provider.
To use this form, select the 'highlight fields' button located at the upper right-hand side of your screen to highlight the spaces where information can be keyed in. The completed form can be saved for copies and for record keeping.

The Disability Services Commissioner will advise disability service providers of their complaints reporting requirements in the future – this template may be used to record complaints data until these requirements are published.
It is envisaged that only deidentified data would be provided to the Disability Services Commissioner as part of the annual reporting requirement.
