If you would like to know more about the Quarterly Data Collection (QDC) Information System, or to help prepare your agency for the new collection, you can view or download the following documents:
QDC Start-up Guide for disability-funded support providers
QDC Start-up Guide (PDF 62 KB) is a quick reference to assist providers new to the QDC information system. This guide complements the QDC Data Guide.
The guide broadly maps out the requirements of the QDC:
- Who is part of the QDC information system
- What data is collected
- How the data can be reported to the DHS.
QDC Newsletter
QDC Newsletter - provides up-to-date information about the QDC information system.
Quarterly Data Collection Data Guide
The guide outlines the data items to be collected and reported as part of the QDC information system. It assists service providers with reporting requirements for the QDC, and those involved in collecting or analysing QDC data.

QDC Data Guide - Addendum 1
Addendum 1 contains the definition of the new Service Type Outlet Question 'S40: Does this service type outlet have an annual quality plan developed in the last 12 months?'
Service User Form
Service User Form (PDF 114KB) (updated 15/09/2009) applies to agencies that have elected to use the paper-based process as their reporting method. The form needs to be completed for each service user who has received a service during a quarter.
QDC User Manual
QDC User Manual (PDF 4760KB) provides a general guide for users of the QDC Tool. The manual includes a detailed step-by-step guide to loading the QDC Tool onto your computer. It also includes detailed instructions on the use of the QDC Tool on a day-to-day basis.
QDC Data Transmission Specification
QDC Data Transmission Specification (PDF 455KB) provides a guideline for software developers and agencies looking to modify their client management software systems to collect the QDC data set. The specification covers:
- The logical hierarchy used in the system
- The XML structure used to construct the QDC export file
- The recommended means of transmitting this file to the department.
It also contains various code excerpts, examples and references to assist in developing a file that conforms to the requirements of the QDC Central Repository. The QDC Central Repository is the department's central storage point for the data collected.
The Data Transmission Specification should always be used in conjunction with the QDC Data Guide, which contains specific detail related to each of the data items covered in the collection.
Third parties modifying software to extract the QDC data from incumbent systems should contact the QDChelp@dhs.vic.gov.au for further information.
QDC software tool training
QDC tool training will be available in the second month at the end of each quarter. For further details you can send an email to the QDC Helpdesk QDChelp@dhs.vic.gov.au to register your interest.
Client consent and information privacy brochures
A series of three brochures can be downloaded relating to information privacy and client consent.
The brochures outline how a client's health and personal information will be collected and handled as part of the QDC. They also offer information on the importance of protecting client confidentiality and gaining consent.
Client Information Brochure (PDF 129 KB) - assists individual service users and outlines the rights of the client and responsibilities of service providers in handling their health and personal information.
