Residential statement
The residential statement is a requirement of the Disability Act 2006. A residential statement is a document for each resident that sets out the residential services to be provided by the disability service provider to support the resident.
Disability service providers must provide a residential statement to a person with a disability when they start living at a residential service.
Disability service providers must use the 
Residential Statement Template (Word 127.0 kb) developed by Disability Services Division.
This template includes the:
- Residential Statement,
- Statement of Rights and Duties and the
- CRU Resident Movement document (only required for residents of community residential units - CRUs).
The Residential Statement Template Completing Instructions (Word 1.1 mb) provides technical information for disability service providers about how to use the template, including how to insert an organisation's logo.

Residential Statement in community languages
The Residential Statement, the Statement of Rights and Duties and the CRU Resident Movement document have been translated into community languages.
The following languages are available in a booklet format. The booklet includes translations of the Residential Statement, the Statement of Rights and Duties and the CRU Resident Movement document.
Facility-based respite residential services
The Facility-based Respite Residential Statement Template (Word 101.0 kb) is to be used by facility-based respite service providers when a person commences using facility based respite services. The template includes the Statement of Rights and Duties.

An easy read version is also available of the 
Facility-based Respite Residential Statement (PDF 3.1 mb).
Duties and Responsibilities - Easy read with pictures (PDF 1.1 mb) would need to be provided with this document to facility based respite service users.
