Our Mission

We do this by:

  • Leading policy and program development

  • Supporting implementation

  • Proactively identifying and prioritising opportunities

  • Evaluating the effectiveness of existing programs and new initiatives

  • Informing and implementing government policy.

    To ensure people recovering their health or with chronic and complex health needs , receive appropriate health care, in appropriate settings, in a timely and co-ordinated way, to achieve the best outcomes in the most cost effective manner.

    Values and Objectives

    The Continuing Care Unit upholds the DHS values, which inform the way we conduct ourselves with internal and external stakeholders.   Part of our performance as a team and as individual staff members will be assessed in accordance with these values.  

    The annual objectives of the Continuing Care Unit's workplan and those of individual staff members are to be assessed for their contribution to

    • the aim of the Unit
    • the mission of the Unit and
    • the wider DHS objectives.
