About us

The main role of the Ambulatory and Continuing Care Section is to ensure the provision of care for older people and people with chronic conditions and complex care needs in a seamless continuum. The aim is to provide policy direction and to assist in the development of appropriate funding and/or targets to enable health services to be flexible enough to provide the right care, in the right place at the right time.
