Grampians' One Stop Shop Open
Access to aged care services will be improved in the Grampians region with the recent launch of Direct2Care. This follows the successful launch of Direct2Care in Melbourne's eastern suburbs in 2008.
The service was launched in the Grampians region by the Minister for Senior Victorians, Lisa Neville and the Federal Member for Ballarat, Catherine King.
About Grampians Direct2Care
Direct2care is about making it simpler and easier for over 32,000 older people who live in the Grampians and their carers to access services.
From Edenhope to Daylesford, and from Ararat to Hopetoun, Direct2Care's Grampians branch functions as a 'one stop shop' for advice about aged care.
Older people, carers or concerned family or friends can phone Direct2Care – 1300 121 121 – and receive one-to-one advice about appropriate aged care services. They can also visit the Direct2Care office at 907 Sturt Street, Ballarat.
After the initial contact with Direct2Care, the support workers will organise appropriate services, such as:
- help with housekeeping
- assistance with showering or dressing
- organised groups with social support activities
- physiotherapy and eye care
- respite services
- food services.
Direct2Care is a joint partnership between the state and Commonwealth Governments. The service aims to help older Victorians find and access the services they need to live fuller, more independent lives in the familiar surroundings of their own home.
This new $2.5 million integrated service will complement current community care arrangements in the region, helping clients and their carers to easily navigate their way and tap into the services they need.
